This guide will open in Adobe Reader.  You can fill out the included forms online while you are in Adobe Reader. 

When you are finished follow these steps:

1. Go to File | Save As to save the document to your computer.
2. Go to File | Print to print the pages you need to turn in.  In the Print Dialog box you'll see the option to print the entire document, or you can print only those pages you'll need to turn in.  Note the page numbers of those pages while you are filling them out and enter those numbers in the box.
3. Send the printed pages to the appropriate office/person.
